There seems to be a problem when loading the last map is checked in the preferences. Even if it is unchecked, a key/value in the registry is STILL added and then BLAMMO! The last map is loaded next time you run the editor. So....

All I'm doing now is running a batch file that deletes that key/value pair in the iw registry directory, and then call COD2Radiant.exe.

I haven't had a problem since doing it this way. Although I have to load my map file manually each time, but hey, it's working now and it's not such a big deal when I'm not getting that error dialog.

My COD2Radiant_Hack.bat file that is in the bin directory with COD2Radiant.exe:
