Aboard a German submarine that was ambushed by Allies. You could have water on the outside of the ship to give the feeling it's underwater.

Have water dripping through pipes. Flickering lights. Steam. Lots of hidden places and pathways.

Maybe even some simple scripted stuff like in the main control room there's a button to press to rise the ship above sea level (make the water brushes go down). Then the players can take the fight on top of the submarine with there being an eagles nest and whatnot.

Man... this would be a fun map, surprised there hasn't been one like it so far.

And also let the control room button be pressed again to go back under water, that way people on top drown when the sub goes back down under water.

ORRRRR... have two subs... one Allies and one Axis... same ideas as mentioned above... but you can use missiles to attack the other ship... so the ships have a set amount of life. This will be something like Titan mode in BF2142.

Ships can rise up but when they are, they can't get hit by missiles nor fire them. But they can use little boats to row across to the enemy's submarine... where they can plant a bomb inside of it if they make it to the control room alive.

Yeah how about a map based in Guadal Canal battles.
You can find plenty of info with google using these islands as your search criteria.
Tanambogo, Gavutu and Gaomi Islands, near Tulagi

I have roughed out the Tanambogo, Gavutu Islands in UO but I am limited with textures and models, Cod2 has what you need for this style of map.
