What does this error mean?  It happens when i try to load a map i made, which is almost empty.  Its just a big room, with one simple patch mesh that has width 9 and height 9, and only happens if I have used the vertex tool make the patch look interesting (if i leave the patch flat, there is no error, but what would be the point of a flat patch mesh?).

my guess - I had this happen too... is that the "bounding box" of you patch mesh is poking out of the top of your skybox... or you've stretched the mesh too much in some way.  do a search on http://www.google.com and you'll find some q3 info on the subject... I did.
