I'm confused here. Your topic is in the CoD4 section, but you are talking about CoD2? The process to make a custom texture in CoD4 is quite a lot different than to make one in CoD2.

This is how I do it for custom textures to be used in Radiant: I save the file in Photoshop in My Pictures (as PSD and TGA or DDS). Then I copy it to raw/textures.
In AssMan, I open my map GDT (which is in model_export for some reason - it doesn't really matter anyway) then check the needed checkboxes, select the file in the colormap section (and in normal and specular map if I have them) and then compile.
The compiled file(s) is(are) in raw/images. These are the files that have to be put inside a IWD, so that you'll see the custom texture when running your map through a mod.

For custom models' textures, I copy the files to model_export/bhd (my mapname). Then open another GDT I've made for custom models (which is in source_data). The rest is the same.

Assman is very buggy, even after the file is deleted assman may insist it is still there. I recommend going into the CoD4 directory and making a folder called texture_assets then creating the texture agian, and attempting it over agian.

First off, you don't save the TGA to GDT. The GDT (Game Data Table) is a file that stores data for assets (materials, models, weapons).
So you make a material (check those checkboxes, put in the name of the colormap and any other maps etc) and you save those settings as a GDT file so that if you change the TGA you won't have to redo those operations. You would just open the GDT (after saving the TGA to the same location as before), select the material and press F10 (convert current asset only).
And you don't have to make a GDT for each material, as it can hold multiple assets.
So, I suggest you to create a blank GDT file first, in texture_assets (as Ownage said), save it, add your material, convert it and save the GDT again. If it still disappears (because I think this is your problem, didn't really understand), then it could be a Vista/7 problem. And double check the save path.
I have XP and never had a problem with AssMan or the ModTools.

Could you post a picture of AssMan with your material selected. I want to see what settings you have.
