In graydiant right-click on the 2d window, in the pop up menu pick "mp", and then select the appropriate spawn or intermission.

Rightclick in the 2d window, mp -> there they are. You need at least one dm_spawn and a dm_intermission for your map to work. You can add more than one spawn point if you want. You'll need spawnpoints for the other gamestyles if you want to play those (tdm for instance).
